DVD/Video Booking Request Form


Use this form to ensure that the DVDs or Videos are available when you need them for a class or special event. The booking request links a specific DVD/Video to your library account, for a specific date and time. Once an item is "booked" for a specific date, circulation that would overlap the booking date & time is not allowed. Plan ahead, and book ahead!

  • NOTE: Booking is available only for current CASE faculty, staff, and students.
  • Search DVDs and videos in our Catalog, to retrieve the DVD or video call number, and then "book" the item for the date you need it.
  • Booking dates can span the normal 7 day loan period. Example: book an item from Wednesday the 1st through Tuesday the 6th, for your Monday 5th class. You can preview it, show it, and return it without overdue fines.
  • Book your item more than one week prior to the date you need it, to accommodate currently circulating items, or late returns by another person.
  • Booking requests can be made online or in person at the Kelvin Smith Library Main Service Desk.
  • Booking requests are processed weekdays only, 9:00 a.m. - 4:00 p.m.
  • Pick up your booked item on the date you specified. Booked items remain in their regular shelving area for security. Simply ask at the KSL Main Service Desk, and staff will retrieve it and check it out to you. Booked items are not delivered through campus mail.
  • All DVDs and videos must be returned on the last day of booking, to be ready for the next user. Overdue fines apply.

Booking privileges are for DVDs and videos only, and is a service reserved for current Case faculty, students, and staff with library accounts in good standing. The Library reserves the right to limit the number of items booked per day for a requestor.

DVD/Video Information

First, find the exact title, edition, etc, and call number on the Catalog. A search for the title Hamlet, "limited" to media, returns 17 items. Specifying a call number guarantees you'll have what you need. Requests without call numbers will be returned to you for selection of format, edition, etc.
